                          A medical scribe is a trained medical professional who specializes in recording and documenting medical information in an electronic health record (EHR) so that physicians can focus on providing patient care. The medical scribe's role is to accurately and efficiently document the physician's patient encounters. This includes collecting and documenting patient histories, vital signs, physical examinations, treatments, diagnoses, and other pertinent information. Additionally, medical scribes help keep the physician up to date with the latest clinical information and technologies, as well as help with patient flow and other administrative tasks. 


                            A medical scribe's primary duties are to follow a physician through his or her work day and chart patient encounters in real-time using a medical office's electronic health record (EHR) and existing templates.
