MEDICAL SCRIBING
Under the direction of a physician, medical scribes, also known as documentation assistants, translate information from clinical visits into electronic health records (EHRs) in real time. Scribing, often known as "team documentation," relieves doctors of the burden of taking notes and filing orders or referrals. This enables medical professionals to concentrate on the patient.
Duties of Medical Scribe :
1. Accurately document patient encounters, including history of present illness, physical exam, assessment and plan, into the medical record.
2. Accurately document a physician’s dictation into the medical record.
4. Research and document medical records from outside hospitals and medical facilities.
5. Accurately document orders and prescriptions into the medical record.
6. Answer patient questions and provide brief explanations as requested by the patient or physician.
7. Assist in the timely completion of patient charts.
8. Assist physicians and other healthcare providers with patient care.
9. Maintain patient confidentiality and protect privacy.
