MEDICAL SCRIBING
A medical scribe is a person trained to document a physician's visits with patients in real-time. The scribe records the information the physician obtains from the patient, as well as the physician's physical exam findings and medical decision-making process. The scribe also assists the physician with data entry into the electronic health record (EHR) system. The scribe ensures that all documentation is accurate and complete.
Medical Scribe Job Responsibilities :
- Prepares and assembles medical record documentation/charts for doctors (s).
- During a patient visit, enters the patient room with the physician to capture and transcribe medical record documentation using electronic medical record applications.
- Self-documentation attestation ensures medical record compliance. Updates the patient's history, physical exam, and other pertinent health information.
- Prepares and sends all documentation to the physician for review and approval using detailed data entry authentication and facility-specific procedures.
- The duration of basic lab results and screening procedures is monitored.
